A sip of cold water.

A spoonful of ice cream.

Hot chai.

Something sweet.

And suddenly you feel a sharp, electric pain shoot through one tooth—or several.

Tooth sensitivity is extremely common, but the cause is not always the same.

Sometimes it comes from exposed dentine near the gumline.

Sometimes from worn enamel.

Sometimes from whitening.

And sometimes sensitivity is your first warning that a tooth has a cavity, crack or deeper nerve problem.

The useful question is not simply:

“Why are my teeth sensitive?”

It is:

“What is exposing or irritating the sensitive part of this tooth?”

Why do teeth become sensitive to hot and cold?

Teeth become sensitive when heat, cold, sweetness or touch can stimulate the dentine beneath the enamel or exposed root surface. Dentine contains microscopic tubules that communicate with the nerve inside the tooth. When these tubules become exposed, temperature changes can produce a short, sharp pain.

The enamel covering the crown normally protects the dentine.

The roots are protected mainly by gum tissue and a thinner layer called cementum.

Sensitivity develops when those protective barriers are reduced.

The seven common causes I see are:

  1. Gum recession
  2. Enamel wear
  3. Tooth decay
  4. Cracks or damaged fillings
  5. Whitening-related sensitivity
  6. Grinding or clenching
  7. Pulp or nerve inflammation

The pattern of pain often helps us work out which one is most likely.

What does normal tooth sensitivity usually feel like?

Typical dentine sensitivity is usually sharp, brief and triggered by something specific such as cold water, hot food, sweet food or brushing near an exposed area. The pain generally stops quickly once the trigger disappears. Pain that lingers, starts spontaneously or becomes increasingly severe needs closer assessment.

A useful distinction is duration.

Cold touches tooth → sharp pain → stops quickly

is different from:

Cold touches tooth → pain continues for 30 seconds, a minute or longer

The second pattern may suggest the nerve inside the tooth is more significantly inflamed.

That distinction matters.

Can gum recession make teeth sensitive?

Yes. When gums recede, the root surface of the tooth becomes exposed. Root surfaces are not protected by the same thick enamel that covers the crown, so temperature and touch can reach the underlying dentine more easily. Sensitivity is often strongest near the gumline.

You may notice this particularly when:

  • brushing
  • drinking cold water
  • eating ice cream
  • breathing cold air

Recession can affect one tooth or several.

Possible contributing factors include:

  • gum disease
  • thin gum tissue
  • tooth position
  • aggressive brushing
  • age-related tissue changes

The cause should be identified rather than simply covering the symptom.

Can brushing too hard cause sensitive teeth?

Aggressive brushing can contribute to gum recession and wear around the neck of the tooth, particularly when combined with an abrasive toothpaste or hard-bristled brush. Over time, exposed root surfaces may become sensitive. More pressure does not mean cleaner teeth; gentle technique is usually more effective and safer.

This is why gum recession from hard brushing matters.

If you repeatedly scrub side to side at the gumline, you may gradually damage the tissues you are trying to keep clean.

A soft toothbrush with controlled pressure is usually enough.

You do not need to polish your teeth like a kitchen surface.

Can worn enamel cause tooth sensitivity?

Yes. Enamel protects the dentine underneath. If enamel becomes thinner because of acid erosion, grinding, abrasion or long-term wear, temperature changes can reach the dentine more easily. Sensitivity may then affect several teeth, especially along biting edges or near the gumline.

Common contributors include:

  • acidic drinks
  • frequent citrus
  • reflux
  • grinding
  • aggressive brushing
  • abrasive toothpastes

One of the most common combinations is acid plus brushing.

If enamel has been softened by an acidic drink and you brush immediately afterwards, wear may occur more easily.

Can lemon water or soft drinks make teeth sensitive?

Frequent acidic drinks can soften enamel over time and increase sensitivity, especially when they are sipped repeatedly throughout the day. Lemon water, citrus drinks, aerated beverages and some sports drinks can all contribute. Frequency and contact time matter as much as the amount consumed.

This does not mean one glass of lemon water destroys your teeth.

The issue is repeated exposure.

A person who sips an acidic drink for three hours exposes the teeth differently from someone who drinks it with a meal and then stops.

Reducing frequency often matters more than banning one specific food.

Can a cavity cause sensitivity to cold or sweet foods?

Yes. When decay progresses through enamel and reaches dentine, a tooth may become sensitive to cold, sweets or temperature changes. Early decay may cause no pain at all, while deeper decay often produces more noticeable symptoms. Localised sensitivity in one tooth deserves assessment.

If the problem is a cavity, sensitive toothpaste will not remove the decay.

The tooth needs proper examination.

You may notice:

  • cold sensitivity on one side
  • pain with sweets
  • food trapping
  • a rough or broken area
  • darkening or visible cavitation

But cavities can also be hidden between teeth.

That is why symptoms alone cannot confirm the diagnosis.

How can I tell sensitivity from a cavity apart from general sensitivity?

General dentine sensitivity often affects several teeth and is strongest around exposed gumlines, whereas decay-related sensitivity is more likely to be localised to one tooth or one area. Sensitivity to sweets, food trapping or a visible defect may increase suspicion of decay, but an examination is needed to confirm it.

Think pattern rather than one symptom.

Several teeth + exposed gumlines + cold: often recession or wear.

One tooth + cold + sweets: decay becomes more suspicious.

One tooth + lingering heat pain: deeper pulp involvement becomes more concerning.

These are clues.

They are not substitutes for diagnosis.

Can a cracked tooth cause sensitivity?

Yes. A crack can allow temperature or biting pressure to irritate the inner structures of a tooth. Cracked-tooth pain can be inconsistent and difficult to locate. Some patients feel pain when biting down, while others feel a sharp sensation when releasing pressure or drinking something cold.

Cracks vary from superficial enamel lines to deeper fractures.

Some are visible.

Many are not.

A cracked tooth may behave normally for days and then suddenly hurt when you bite on exactly the wrong angle.

That unpredictability is common.

Persistent localised sensitivity deserves evaluation, especially if you also notice pain when chewing.

Can an old filling cause sensitivity?

Yes. Fillings can wear, fracture, leak or develop decay around their margins. A tooth may then become sensitive to temperature, sweets or biting. Recent fillings can also cause temporary sensitivity while the tooth settles, but persistent or worsening symptoms should be checked.

After a new filling, mild sensitivity for a short period can happen.

The important question is the trend.

Improving each week: reassuring.

Becoming more intense: needs review.

Pain when biting: may indicate the bite needs adjustment or another issue is present.

Lingering heat or cold pain: the nerve should be assessed.

Can teeth whitening cause sensitivity?

Yes. Whitening products can temporarily increase tooth sensitivity because peroxide can pass through enamel and dentine and irritate the nerve response. This usually settles after treatment stops or the whitening schedule is adjusted. The effect is more likely in people who already have exposed dentine or recession.

Whitening sensitivity is usually:

  • temporary
  • generalised
  • linked clearly to whitening sessions

If one tooth becomes dramatically more painful than the others, I would not automatically blame the whitening product.

That tooth may have an underlying problem that the whitening has simply revealed.

Whitening should ideally be started after checking that the teeth and gums are healthy.

Can grinding or clenching make teeth sensitive?

Grinding and clenching can contribute to sensitivity by creating enamel wear, microfractures and repeated stress on teeth. Over time, this may expose dentine or make individual teeth tender. Some people are unaware they grind until wear patterns, morning jaw fatigue or sensitivity begin to appear.

Common clues include:

  • flattened tooth edges
  • chipped enamel
  • jaw tiredness in the morning
  • headaches
  • repeated filling fractures
  • unexplained sensitivity

Not every grinder needs treatment.

But if grinding is damaging the teeth, protecting them becomes important.

The cause of grinding may also need to be considered.

Can gum disease cause sensitivity?

Gum disease can contribute indirectly by causing gum recession and loss of supporting tissue, exposing sensitive root surfaces. Inflamed gums themselves usually cause bleeding and tenderness rather than classic cold sensitivity, but recession associated with periodontal disease can make root sensitivity much more noticeable.

This distinction helps.

Bleeding gums are primarily a soft-tissue sign.

Cold sensitivity is usually a tooth/dentine response.

But both can occur in the same area when periodontal disease has exposed the root.

Treating the gum disease matters more than simply applying desensitising toothpaste.

Can sensitivity after professional cleaning be normal?

Yes. Temporary sensitivity can occur after scaling or deep cleaning, particularly when calculus has been removed from exposed root surfaces or inflamed gums begin to shrink. It usually improves over several days or a few weeks. Persistent or severe sensitivity should be reassessed.

A professional cleaning does not create disease.

Sometimes it exposes surfaces that were previously covered by heavy calculus.

If the root was already exposed underneath that calculus, you may notice temperature sensitivity once it is removed.

Desensitising treatment and time often help.

If you notice sensitivity after scaling, it is often temporary and related to exposed root surfaces or areas that were previously covered by calculus.

When does sensitivity mean the nerve inside the tooth is inflamed?

Sensitivity becomes more concerning when pain lingers after hot or cold stimulation, occurs spontaneously, wakes you at night or becomes increasingly intense. These patterns can indicate inflammation of the dental pulp rather than simple exposed dentine. The tooth should be assessed promptly to determine whether the nerve can recover.

Short pain and lingering pain are clinically different.

A quick cold “zing” often suggests dentine sensitivity.

Pain that keeps throbbing after the cold has gone can indicate deeper pulp inflammation.

Heat sensitivity can also be significant.

If the pulp becomes irreversibly inflamed or infected, root canal treatment may be required.

When might sensitive teeth need root canal treatment?

Root canal treatment may be needed when sensitivity is caused by irreversible inflammation or infection of the pulp rather than exposed dentine alone. Warning signs include prolonged pain after temperature changes, spontaneous throbbing, pain waking you at night, severe heat sensitivity or symptoms associated with deep decay or fracture.

Not every sensitive tooth needs a root canal.

Far from it.

Most generalised sensitivity does not.

The goal is to determine whether the nerve is simply irritated or permanently damaged.

That requires clinical testing, and sometimes X-rays.

Root canal treatment should be based on diagnosis, not on sensitivity alone.

Does sensitive toothpaste actually work?

Desensitising toothpaste can help when sensitivity comes from exposed dentine. Ingredients such as potassium salts, stannous fluoride or other desensitising agents can reduce nerve response or block dentinal tubules. Consistent use is important, and improvement may take several weeks rather than one or two applications.

To get the most benefit:

  • use it consistently
  • brush gently
  • avoid rinsing aggressively afterwards
  • apply a small amount directly to a very sensitive area if your dentist recommends it

But remember:

Sensitive toothpaste treats sensitivity.

It does not treat decay, cracks or an infected nerve.

How long does sensitive toothpaste take to work?

Desensitising toothpaste often needs several weeks of regular use before its full benefit becomes noticeable. Some people improve sooner, while others need four to six weeks. If symptoms are unchanged, worsening or limited to one specific tooth, the cause should be assessed rather than simply continuing indefinitely.

This is particularly important if you keep buying stronger sensitivity products without knowing why the tooth hurts.

If there is a cavity under the enamel, toothpaste cannot fill it.

If there is a crack, toothpaste cannot seal the fracture.

Treatment must match the cause.

What else can I do at home for sensitive teeth?

Use a soft toothbrush, avoid aggressive scrubbing, reduce frequent acidic drinks, use a suitable desensitising toothpaste and avoid extreme temperature triggers while the area settles. If you grind your teeth or have gum recession, those problems may need specific management rather than home treatment alone.

A sensible starting routine is:

  1. Soft-bristled toothbrush
  2. Gentle pressure
  3. Fluoride/desensitising toothpaste
  4. Reduce frequent acidic drinks
  5. Do not brush immediately after acidic foods or drinks
  6. Avoid repeated whitening if symptoms are active
  7. Arrange an examination if sensitivity persists

Simple habits can make a large difference when dentine exposure is the cause.

Should I stop drinking cold water if my teeth are sensitive?

You do not necessarily need to avoid cold drinks permanently. Temporarily reducing very cold triggers can improve comfort, but the real goal is to identify why the sensitivity exists. If normal cold water consistently causes sharp pain, particularly in one tooth, the underlying cause should be assessed.

Avoidance can hide the symptom.

It does not solve the problem.

If you have switched entirely to room-temperature water because one tooth hurts every time something cold touches it, that is useful information.

Tell your dentist exactly what triggers the pain and how long it lasts.

Can sensitive teeth heal on their own?

Temporary sensitivity from whitening, recent cleaning or mild irritation may settle on its own over days or weeks. Sensitivity caused by exposed roots, enamel loss, decay or cracks usually does not permanently resolve without addressing the underlying cause, although desensitising treatments can reduce symptoms.

The word “heal” matters here.

Dentine sensitivity can become less noticeable.

But lost enamel does not simply regrow.

A cavity does not repair itself once it has progressed beyond the earliest mineral changes.

A crack does not fuse back together.

Persistent sensitivity therefore deserves an explanation.

When should I see a dentist about sensitive teeth?

See a dentist if sensitivity is new and localised to one tooth, persists beyond several weeks, becomes stronger, lingers after hot or cold stimuli, causes pain when biting, wakes you at night or occurs without an obvious trigger. These patterns are more likely to need treatment than mild generalised sensitivity.

I would be especially interested in:

  • sudden onset
  • one specific tooth
  • lingering pain
  • heat sensitivity
  • night pain
  • chewing pain
  • swelling
  • a broken tooth or filling

Those features can point away from simple dentine sensitivity.

Early assessment often means simpler treatment.

What treatment can a dentist provide for sensitive teeth?

Treatment depends entirely on the cause. Options may include fluoride or desensitising applications, modifying brushing habits, treating gum disease, repairing cavities or cracks, replacing defective restorations, protecting teeth from grinding or providing root canal treatment when the pulp is irreversibly inflamed.

That is why “What toothpaste should I buy?” is sometimes the wrong first question.

Sensitivity is a symptom.

The diagnosis determines the solution.

Two patients with identical cold sensitivity may require completely different treatments.

One may need only desensitising toothpaste.

The other may need a filling.

What should I remember about sensitivity to hot and cold?

Brief generalised sensitivity often comes from exposed dentine, recession or enamel wear, while sudden localised or lingering pain deserves closer assessment for decay, cracks or nerve inflammation. Sensitive toothpaste can help dentine sensitivity, but it cannot treat every cause. The pattern of pain is one of the most useful clues.

The simple rule is:

Short, generalised sensitivity is usually less worrying than pain that is localised, lingering or worsening.
